To help decrease appointment wait times and make communicating with our office a little easier.
In an effort to ensure that your services are covered, we want to remind you that if your insurance company requests a referral, or an authorization for any specialist’s services, they are obtained prior to your scheduled appointment. Please notify our staff of any insurance changes that may have occurred after your appointment has been scheduled in order to avoid being billed directly. Thank you for helping us serve you better.
PLEASE remember to bring your insurance card(s) and a picture ID to each visit.
PLEASE remember to be prepared to pay all copays and deductibles at the time of service. We accept check, money order & cash along with credit cards payment with 4% convienence charge.
PLEASE remember to call the office 48 hours in advance if you are not able to make you scheduled Testing or Surgery appointments in the Canton office. There is a $50 fee for testing no shows and $150 fee for surgery no shows.
PLEASE remember that we DO NOT ask for any payment for testing until after your insurance is billed. Per your plan, you may have a balance or a copay due outside of your normal office visit.
PLEASE remember to utilize our patient portal at My Doctor (myezyaccess.com). The portal is a secure way that enables our team to communicate directly with you. You are also able to fill out paperwork, pay bills on line, access your health information, request refills, request appointments and ask non-emergency questions.
